Real Estate Lead Form Template for Buyers, Sellers and Renters
One form, three paths. The first question asks what the visitor wants to do, and the form shows only the questions that matter for that goal. Your agents get a lead they can act on: budget, area, timeline and how to reach them.
Step 1: One question splits the form
What would you like to do? Everything after this depends on the visitor's choice.
The Buyer Path
Shown only to prospective buyers looking for residential or commercial properties.
| Question / Field | Field Type | Why Ask It |
|---|---|---|
| Property type | Choice: apartment, villa, townhouse, plot | Filters listings before the first call |
| Preferred areas | Location field, multiple areas | Matches the lead to the agent who covers that area |
| Budget | Bands, e.g. Under $500k / $500k–$1M / $1M+ | Bands get more answers than an open number |
| Bedrooms | Single Choice (Studio, 1, 2, 3, 4+) | The other filter every buyer search needs |
| When do you want to move? | Now · 1–3 months · 3–6 months · Just looking | The best single signal of lead quality |
| Finance status | Pre-approved · Need mortgage advice · Cash | Tells you who can buy now |
The Seller Path
Shown only to property owners requesting a home valuation or listing proposal.
| Question / Field | Field Type | Why Ask It |
|---|---|---|
| Property address | Address autocomplete + map pin | An exact address for the valuation visit |
| Property type and size | Choice + number (sq ft or m²) | Lets the agent prepare comparables |
| When do you plan to sell? | Immediately · 1–3 months · 3–6 months · Exploring | Prioritises the valuation queue |
| Is it listed with another agent? | Yes / No | Avoids wasted duplicate calls |
| Would you like a free valuation? | Yes / No | The offer that turns browsers into appointments |
The Renter Path
Move-in date · Monthly budget · Preferred neighborhood · Length of lease · Pets (yes/no) · Number of occupants.
Last Step for Everyone
Full name · Email address · Phone number · Best time to call · WhatsApp or phone call preference · Consent to contact.

Tips for This Form
- Ask the timeline early. It sorts hot leads from browsers better than any other question.
- Never ask buyers for their home address. Ask for the areas they want instead.
- On listing pages, use a short version: 'Book a viewing' with date, time and contact only.
- Show a map for sellers. A pinned location removes 'which building?' confusion calls.
Where to Place It
- Homepage hero: 'What would you like to do?' as the first visible interactive question.
- Every property listing page: short viewing request version.
- Neighborhood guide pages: buyer path with that specific area pre-selected.
- 'What's my home worth?' landing page: seller valuation path only.
Property & Construction FAQs
Goal (buy, sell or rent), area, budget, property type, timeline and finance status, then contact details. Ask only the questions for the visitor's goal.
